Regarding Transwarp:

It never worked. Excelsior was supposed to be the test. According to Mr. Scott’s Guide to the Enterprise, even after they fixed his meddling it never worked.

Okuda just said good thing it didn’t work and mentions the Voyager episode where when they try transwarp for the first time Janeway and Paris turn into lizards.
