That quote gave me the heebee jeebies the first time I saw it. It was spooky and weird and the first real idea you had that these “visions” were thinking purposeful beings and not just some wacky projection or shared halucination. At least that’s how I remember feeling about it.
Aren’t we all? Guys…? I love the idea that on a magical island where all injuries are healed a guy gets cancer on the same day that a surgeon literally falls out of the sky.

I wondered that exact same thing but I think it might have been something else. From memory the actor deciding to leave happened after the end of s2 but before s3. I think that story line was already well in motion by then. If not obvious to us then at least to the writers who had the plans.
By inference you are now up to season 3 episode 8 “Flashes before your eyes”. Reminds me of a discworld moment when:
